While our goal at the Ohio Valley Center for Periodontics & Implants is to always provide exceptional customer and patient service, please be aware that all professional services rendered are charged directly to the patient, and that the patients are personally responsible for payment of fees. As we cannot guarantee dental benefits and reimbursement at any level, please know our office does offer a Complimentary Benefit Analysis in order to accurately and efficiently determine financial solutions. All accounts are due and payable at time of services rendered, unless prior arrangements have been made. We do not render our services on the basis that insurance companies will pay our fees. As a courtesy, we will process and submit any necessary dental insurance paperwork, however all unpaid balances left after insurance are the full responsibility of the patient.

So that we may assure you and other patients of uninterrupted treatment, it is necessary for all patients to accept and adhere to a plan of care and financial treatment plan. Please be courteous to the fact that you have reserved time in the schedule, and if at any time you feel you are unable to keep your scheduled appointment, a 72 hour cancellation notice is required. In instances where proper cancellation notice is not given, a non-refundable fee may apply. Failure to notify us in a timely manner denies other patients the opportunity to complete their surgical care.

For your convenience, we accept all major credit cards, cash and check. In some instances, extended payment options are available with approved credit through outside, third party agencies.
