what information do i need to reserve an appointment?

We want your experience to be effortless. By providing our scheduling team with the necessary information at the time of scheduling, we are able to seamlessly expedite your appointment upon arrival. When you are ready to schedule your appointment, having the following information is much appreciated:

  • Your referral information.
    • This may be provided by your general dentist. If you don’t have a referral, don’t worry – explaining your current dental issue will allow us to properly reserve an appointment time.
  • Dental benefits information.
    • As a courtesy, we provide a complimentary benefit analysis prior to your initial consultation. Having this information on the front end allows us ample time to verify your coverage. In most cases, we need the policyholder’s name, date of birth, and SSN or Member ID that may be printed on the dental benefits card.

what happens at my initial consultation?

Upon reserving your initial consultation with Dr. Silverstein, you will be provided with an in-depth Patient Registration Form.  For your convenience, you may fill these forms out on our website by visiting our Patient Registration Page. We are also happy to mail the forms to your residence or provide them via email. 

Prior to your initial consultation, we will reach out to your General Dentist and request any recent x-rays or images be electronically sent to us. Our office offers a wide scope of digital and state-of-the-art technology. In cases where x-rays may not be available or relevant to your treatment needs, we may recommend additional diagnostics be completed at our office.

Our Dental Benefits & Finance Coordinator will use the dental benefit information you originally provided to complete your complimentary benefit analysis. This information may be applicable if a treatment plan is determined during your appointment.

If you take a pre-medication as the result of a previous medical condition or concern, please contact our office prior to your appointment. Your health and safety are very important to us, and taking the proper precautions is always key.

At your initial visit, Dr. Silverstein will complete an in-depth medical and dental health history and a thorough examination to evaluate bone loss, loose teeth, missing teeth, occlusion(bite), oral cancer screening, TMJ, esthetic considerations (appearance), and other signs of periodontal disease. You may also meet with a member of our Clinical Team or a Treatment Coordinator. To help with financially related questions or complete your scheduling requests, a Financial Coordinator and a Scheduling Coordinator will be available to you.

As part of our commitment to the team approach to dentistry, our goal is to have a close correspondence with your general dentist. It is imperative they remain a primary resource in your journey to dental health.

what happens once my treatment has been completed?

Meet Dr. Silverstein delivers ongoing support throughout his patients’ dental journey, which continues after their treatment has been completed. He follows them closely and provides continuing care with his Surgical Assisting Team and Registered Dental Hygienists who maintain patients who have suffered from or have been treated for gum disease.
